...Porky Minch in MOTHER 3 is almost scarier than Gigyas in MOTHER 2 (he's that guy who was alongside Gigyas at the beginning), truth be told 
I would say the ReDeads from Zelda are pretty much the scariest thing I've every seen. (I don't play a whole lot of FPSes, etc., so...)
2 – Masterge77, AuraKshatriyaIf it were me, it would either be The Immoral Beast or The Child from Catherine. Don't get me wrong, the entire game is revolved around nightmares, so it's obvious that the bosses would be scary, and the child is literally a giant, disfigured baby doll that keeps calling you "Daddy," but the Immoral Beast... I can't even describe it in words. Just look them up on a wiki or something and you'll know what I'm talking about.

Anyone gonna join me in saying pretty much every enemy in Minecraft (ex. Slimes)? I've almost had that game a year, I still almost piss myself when a creeper/zombie/spider/skeleton comes out of the dark suddenly.
4 – zsaberslash, Shiny, 7dewott8, 11Trident11Chozo Ghosts in Prime 1, Dark Samus's deteriorating form in Prime 2, any iteration of Nightmare (Fusion or Other M, but Other M is scarier in my opinion), Wallmasters and Floormasters from the Zelda titles, Tanks from L4D2, the various forms of Ballos from Cave Story, Phantoon from Super Metroid and from Other M, Endermen from the upcoming Minecraft 1.8 (although I'm starting to get used to them)

2 – Masterge77, alternateshadow300Well the only time that enemies truly scared me was when I was younger, and I never really played many horror games. Owning Majora's Mask as a little kid certainly helped when it came to being scared of video game enemies, especially the Gibdos in Ikana Valley, ESPECIALLY because of the music there (scary stuff for a 7-year-old). The scientist in the musicbox house that got turned into a Gibdo, though not really an enemy, was also pretty scary since he appears by complete surprise, mini-boss music plays, and he looks and sounds plain frightening.
Other than him, the Water Wraith from Pikmin 2 is all I can really think of, because they come by complete surprise, can not be killed, moan terribly, and reside in one of the eeriest caverns. It also does not helpmthat I was mediocre at Pikmin 2 when I first got it, adding to the death toll.
Well, not exactly a villain, but fake Colonel from MGS2 freaked me out, telling me to turn off my game, turning my radar into a woman, morphing his face to look like a zombie, two out of those three things happening during the main character was naked in every sense of the word, which just added to the "creepy" and "Wtf" factors.
Oh. Also. The first Uroboros agent you meet in RE5. I forgot his name.
Actually, ever Uroboros except Wesker.

First one is Marx Soul from Kirby Super Star Ultra. Kirby games are pretty famous for having pretty creepy final bosses, but this is by far the scariest boss I've ever encountered. Seriously, as if Marx wasn't already demented enough in the original, they decide to make him even more nightmarish in the DS remake. Especially when he's defeated. (And I was playing the game late at night, too... D: )
Secondly, Perfect Dark Gaia from Sonic Unleashed. I mean, it doesn't really scare me (No video game bosses really scare me nowadays), it's just a really creepy boss. And it's practically the only reason the game got an ERSB rating of E10+, and it also notes on the back cover it has animated blood. This is because Dark Gaia literally leaks green blood during its transformation to Perfect Dark Gaia, and this is also seen when it's defeated. Aside from that, it also has a pretty creepy design.
I mean, just look at it...
3 – FreezeWarp, zsaberslash, Masterge77[I don't play a lot of video game series, but when I do I tend to know them inside and out. Zelda is an example. (Others: MOTHER, Mario 3D, Mario Side Scroller, Paper Mario, Pokémon, Professor Layton)]

Another enemy that frightened me when I was little was Headlesses, from Ultima Underworld:

Especially after seeing their horrifying picture in the game's manual:

1 – Masterge77I may be one of the few who has actually played the Ghostbusters game for the Wii, but there's this haunted hotel level with a layout similar to the Lost Woods in Oot, but the rooms are much smaller. There are these ghosts in the level that look like pitch-black versions of people with glowing red eyes, and in a hotel where all the lights are cut and I can't see around the corner, when they jump out and lash at me, it, with lack of a better term, "Scares the **** out of me." Plus, they have a ton of health and you can't defeat them without using the unreliable shotgun, if you don't want to be murdered.
